 These Terms of Use (“Terms”) govern your access to and use of the ReNova Wellness website and services. By accessing this website, enrolling in a membership, or receiving services from ReNova Wellness (“Practice,” “we,” “us,” or “our”), you agree to be bound by these Terms. 

Use of this website alone does not establish a provider–patient relationship. A provider–patient relationship is established only after completion of required intake forms, informed consent, medical evaluation, and acceptance into care by a licensed provider.

2. Medical Disclaimer

Content on this website is provided for informational purposes only and does not constitute medical advice, diagnosis, or treatment. Medical decisions are made only after a clinical evaluation by a licensed provider.

3. Eligibility for Services

To be eligible for services, you must:

• Be at least 18 years of age
   
• Be physically located in a state where ReNova Wellness providers are licensed at the time of service
   
• Complete all required intake and consent forms
   
• Be deemed clinically appropriate for care
   

ReNova Wellness reserves the right to decline, suspend, or discontinue services at its discretion when medically appropriate or if these Terms are violated.

4. Telehealth Services

You consent to receive healthcare services via telehealth, which may include video, phone, or electronic communication. You acknowledge that:

• Telehealth has limitations compared to in-person care
   
• Technical issues may occur
   
• Telehealth is not appropriate for emergency care

5. Membership and Pricing Structure  (Blended Model) 

ReNova Wellness operates under a monthly membership-based care model.

Membership fees:

• Cover professional medical services, provider time, and ongoing treatment management
   
• Include treatment provided under an individualized care plan, which may include medication
   
• Are not based on medication quantity, dosage, refill timing, or outcomes
   

Membership fees are charged monthly and remain due regardless of treatment adjustments, pauses, or changes.

Pricing is subject to change with reasonable notice.

6. Medication and Treatment

When clinically appropriate, treatment plans may include prescription medication.

You understand that:

• Medications and dosages are determined by licensed providers based on medical necessity
   
• Treatment plans may change over time
   
• No specific medication, dosage, refill schedule, or treatment outcome is guaranteed
   

Medication inclusion within the membership reflects the overall cost of care and treatment management and does not constitute the retail sale of medication.

7. Payments and Billing  Authorization 

By enrolling in a membership, you authorize ReNova Wellness to:

• Charge recurring monthly membership fees to your payment method on file
   
• Continue billing until proper cancellation is submitted
   

All membership fees are non-refundable once billed, except where required by law or expressly stated in writing.

It is your responsibility to maintain valid payment information and to cancel prior to the billing date to avoid future charges.

8.  Refund Policy

All payments are non-refundable once billed.

No refunds will be issued for:

• Partial months
   
• Missed or unused services
   
• Treatment or medication changes
   
• Dissatisfaction with outcomes

9. Privacy and HIPAA

 Your personal health information is handled in accordance with our HIPAA Notice of Privacy Practices and Privacy Policy, which are incorporated into these Terms by reference. 

10. Intellectual Property

All website content, branding, and materials are the property of ReNova Wellness and may not be reproduced without permission.

11. Limitation of Liability

ReNova Wellness makes no guarantees regarding treatment results or outcomes, including weight loss. To the fullest extent permitted by law, liability is limited to the amount paid for services during the applicable billing period. 

12. Modifications To Terms

ReNova Wellness reserves the right to update or modify these Terms at any time. Updated Terms will be posted on the website and apply prospectively.

Continued use of services constitutes acceptance of revised Terms.

13. Governing Law and State Restrictions 

These Terms are governed by the laws of the state in which services are rendered, including Texas and Louisiana, without regard to conflict of law principles.

ReNova Wellness is legally authorized to treat patients located in Texas and Louisiana only at the time of service.
